list of survey look like that:
Name, Email, View Survey
Admin panel: can create/edit Categories (category have name and desc)
can create/edit questions. question can have 4 types: qty scale yes/no text box (one line)
form with details in the bottom (all mandatory except the message field)
you can order the categories.
